Introduction to the Conversation Project: Helping People Share Their Wishes for Care Through the End of Life (1.5 hr)
This session helps participants explore how to talk about their healthcare preferences and support their wishes being honored. Topics include communicating end-of-life wishes with people who should know, advance care planning, building a support network, naming a health care agent for the Medical Durable Power of Attorney (MDPOA), and accessing community resources. Whether you're just beginning the conversation or updating your plans, this session offers practical guidance and a space to reflect on what matters most.
